Pagosa Springs Medical Center has an opening for a Full-Time Emergency Medicine Physician.  Must be Board Certified/Board Eligible in Emergency Medicine or have extensive Emergency Medicine experience. Full-time is generally 10 to 12 shifts per month with an equitable mix of days, nights, weekends and holidays. Job Duties

This position provides integrated behavioral health care to Rural Health Clinic patients.  This type of care is delivered with a warm hand-off during the clinic day or is a scheduled visit. This position also provides typical social work supportive to patient needs. The IBHCP uses specialized education, discretion and judgement to perform the following: 1. Assess the patient’s physical, psychosocial, cultural and spiritual needs, utilizing evidence-based screening tools (PHQ9, GAD7, CSSRS, etc.); 2. Assist in the coordination of appropriate services, resources, and referrals; 3. Advocate and provide support to patients and families having difficulty coping effectively with changing medical conditions and other social determinants that may affect their health and well-being; 4. Diagnose and provide short-term evidence-based behavioral health therapy in the outpatient Rural Health Clinic; 5. Refer to a higher level of care when required for diagnosis and medication management; 6. Facilitate remote therapeutic sessions with offsite providers (Psychiatrists, Psychiatric NP, etc.) via telehealth when needed and act as a liaison between the specialist and the primary care provider. Duties may be changed as clinic needs are modified.

Job Requirements

    • Graduate of Accredited Program corresponding to licensure.
    • LCSW, LPC, or LMFT license required in Colorado.
    • Beneficial to have two (2) years social work experience in a hospital or similar Clinic setting; referral to community resources; discharge planning; and/or case management or other related experience.
    • Knowledge of community resources, hospital operations and excellent communication/presentation skills.

Operates clinic telephone switchboards and consoles to connect, hold and transfer telephone calls and provide information to assist callers or refer them to appropriate contacts, either in the organization or elsewhere. The Call Receptionist does daily problem solving for incoming callers by making informed decisions by communicating with the proper department, as well as checking and confirming patients’ appointments. The Community Health Advocate (CHA) position requires an individual that has strong interpersonal skills that can connect with patients without judgement. This 1:1 relationship involves the screening for social needs (i.e. Food, Housing, Transportation, Utilities, Finances, Personal Safety). These elements are called the Social Determinants of Health (SDoH). The CHA will use a standardized screening tool and develop a methodology for delivery, evaluation, and follow-up.  Positive screens will alert the CHA of the opportunity to provide tangible resources to the patient and/or family. CT/RT Technologist

The Radiologic Technologist will support the mission and values of PSMC. Under the direct supervision of the Diagnostic Imaging Manager, the CT/RT Tech performs radiographic procedures to assist in a diagnosis of disease and injuries. The CT/RT Tech also performs fluoroscopic procedures to assist the Radiologist, Surgeon or ordering provider in various locations in the facility. The Infusion Services Nurse Manager is responsible for the direct oversight and management of personnel assigned to the outpatient Infusion services area. Under the direct supervision of the Diagnostic Imaging Manager, the Ultrasound Technologist performs diagnostic sonograms as requested by Providers. The Ultrasound Technologist selects appropriate transducers, depending upon depth of field, organ or body part to be identified, etc. The Ultrasound technologist conducts scanning procedures using ultrasound equipment and assists the physician with procedures, such as biopsies and aspirations. The Ultrasound Technologist works effectively with patients and patients’ families and maintains a cooperative relationship with interdepartmental personnel.   • Education/Experience Requirements: Must have completed course in Sonography and Vascular technology, and may have additional registration as Registered Diagnostic Medical Sonographer (ARDMS). Sonographer must maintain Continuing Medical Education as required by registry. Minimum three (3) years clinical experience preferred.
  • Special Skills, Licenses or Certifications:
    • Must be registered through ARRT or ARDMS with certifications in OB/GYN, Abdomen and
    • RVT preferred.
    • Basic computer knowledge.
    • Typing/word processing/excel/email.
    • BLS/CPR
