Pagosa Springs Medical Center offers our patients a convenient and secure website and app to help manage your health. HealtheLife connects you with your care providers, lets you check test results, and view personal health information.
Once you have an account set up, you can access HealtheLife anywhere you have access to the internet or from the HealtheLife app on your smartphone.
If you have forgotten your username or password, go to https://pagosasprings.iqhealth.com and click on the “Forgot Password?” to reset your password. If the issue persists, please call 877-621-8014 24 hours a day, 7 days a week.
Please email us at Informatics.Dept@psmedicalcenter.org and request that we change the email address associated to your existing portal account. Please provide your name, date of birth, the new email address and the last four digits of your social security number so that we can send you a new invitation. Please note that it takes 24 hours for the current account to be disassociated from the current email address so you will receive the invitation at your new email address within 2 – 3 business days.
If the child is 0 – 13 years of age, you may add the child to your own portal account by emailing Informatics.Dept@psmedicalcenter.org and requesting that we add the child to your portal account. Please provide your name and date of birth, your child’s name, his/her date of birth, the last four digits of the his/her social security number, and the email address used for the parent’s portal account.
If the child is 14 – 17 years of age, the child will need to contact us directly requesting their portal account that utilizes their own unique email address. In order to comply with a number of privacy laws, PSMC does not allow a Teenage Minor Patient Ages 14-17 to create a portal with the use of any other person’s email address and does not allow parents/legal guardians to be authorized representatives on the portal created by the teenage minor patient. Patients over the age of 18 are not minors and not subject to these limitations.
Yes. HealtheLife is HIPAA compliant and provides you with the ability to securely view, store, and share your health information.
Cerner is the company PSMC uses to securely manage your electronic medical record information. HealtheLife is a customized Cerner system which provides you with the capabilities to interact with the medical record system.
Apple Health works with your HealtheLife Portal to offer another way to securely connect to your health information. Pagosa Springs Medical Center may be able to connect other vendors, if requested. Please keep in mind that technical requirements may prohibit some connections and the vendor engagement process is often lengthy.
